Membership Pricing

All membership fees are per month. Preventive, chronic and acute care are included with no additional copay or cost for each visit. There are more details on what is included in your membership below.

*First 2 children are at the listed rate, each additional child is $10 off.

  • There is a one time sign up fee of $50 for individuals and $75 for families.
